Liability waiver

Please review this participation waiver before registering your child for Warriors Athletics.

This sample waiver is provided for program planning and should be reviewed by a qualified attorney before it is used for enrollment.

Assumption of risk

I understand that participation in Warriors Athletics after-school flag football and fitness activities includes inherent risks, including falls, collisions, strains, sprains, and other injuries. I voluntarily assume these risks on behalf of my child.

Release of liability

To the fullest extent permitted by law, I release Warriors Athletics and its coaches, volunteers, host locations, and representatives from claims arising from my child’s participation, except where caused by gross negligence or intentional misconduct.

Parent responsibilities

I agree that my child will follow coach instructions, use equipment as directed, and participate respectfully. I will share relevant medical, accessibility, or safety information with Warriors Athletics before participation.

Program changes

I understand that Warriors Athletics may need to modify activities, pause play, or cancel a session due to weather, safety conditions, or host-location guidance.

Parent acknowledgment

These acknowledgments are included in the registration process.

I understand that flag football and fitness activities involve physical activity and a risk of injury, and I voluntarily allow my child to participate.
I confirm that my child is medically able to participate in the program, or that I have consulted an appropriate healthcare professional.
I agree to follow Warriors Athletics safety rules, coach instructions, and host school or field procedures.
I authorize emergency care for my child if I cannot be reached, understanding that I am responsible for related costs.
